I have been on a wah history lesson of sorts over the last few weeks and came across the theme song from Shaft. The article here states that Isaac Hayes was down on his hands and knees working the wah.

In case anyone else is interested I found out it was Boomerang Maestro. Looks like the company that made these at the time was a company owned by Gibson.
