This is a Charvel San Dimas II. Look below the Charvel logo on the headstock to see where it's made

This serial could also belong to a Charvel San Dimas reissue

That's what it appears to be, one of the "reissue" models. San Dimas II (just with a three-way toggle switch instead of the blade switch). They made them with both switches for some reason.
