It's the AT or AT1 Headstock on the Custom Shop form, and SLS headstock in just about all other literature.

My NAMM SLS wall card has it listed as "AT1 3x3 head"


And for the last time, it is NOT a Smurf Hat. ESPee and Craparison use the Smurf Hat. If anything, the SLS head is closer to spear tip than a Smurf Hat.
