Danelectro guitar help needed fast!!

I have been looking for a vintage Dano 12 for a long time and this one looks like the one I want. It is a refin but I don't know anything else about it. I checked the Dano vintage site and the headstock shape is not shown. Does anybody know anything about this guitar??

It appears to be a 1965 model 4011 minus the pickguard & tremelo arm.
