Model 4 wiring ?

Can anyone help me out ? I just bought a Blue Model 5 in decent shape but the seller failed to tell me that it's not wired to work. The input jack is disconnected electrically as is one of the pickups and I'm not sure which one. A wiring diagram will get this this back in the game. Thanks in advance.

It shows the mid boost circuit. On the right is the switch set ups, you're looking for the one with three mini switches.
