it looks a bit like a CHS1, except is has the top-mounted jack and an extra tone knob. But the pickguard may have been changed, and the 2nd tone knob added.

If it is a CHS1, recommended retail was $345 according to the '95 catalogue

It looks like a CS-20 i picked up at a yard sale a while ago. EXACT same setup. I paid $50.00

Actually it's the CS-10. The CS-20 is the HSS config.
