PSA: Who Owns SLATQH JA349?

I used to own this guitar but sold it on ebay a few years back...I would guess around 2006. I was going through a box full of guitar parts the other day and came across the hang tag. If anybody here owns it or knows who does, I'll be happy to send them the tag.

Here's the guitar, but the buyer wanted the original pickups back in it so when it left my possession the pickups and rings were black.
