Tremolo arm keeps coming loose & falling off

My newest guitar is a Jackson DK2 with chrome hw. Just practicing at home, the tremolo arm will get loose, no matter how hard I tighten it. Playing live, forget about it - it will fall off. My only trem I have ever had this issue with. Any fixes?

peen the arm nut. or loctight.
