How to rewire a Model 6 without the active boost?

I have a Model 6 that I bought without the active boost. I am planning on installing some new mini humbuckers in the neck and middle positions.

The thing is that I want to wire it so that there is a master volume and then a tone for the neck and another tone for the middle.

While I know how to wire up the individual three way switches I am stumped as to how I can get that volume/tones setup since there is no standard 5-way switch like say a Strat.

Suggestions?

I really hate the active bit but did not want to just stick a dead pot to fill that last hole. Now I'll have a totally functional guitar!

No problem. I really don't care for the mid boost circuit either, and this was a diagram I got somewhere off the net a few years back. I used it for a model 4.
