Help with a Model 5 switch wiring

I just bought a Model 5 and the switch was wired incorrectly and some wires were broken off. I need any help putting it back to original. I used the diagram on the jacksonmuseum site and had no luck. It works, but not correctly. If someone has a diagram or can let me call and discuss where the wires go, it would be greatly appreciated. Thanks

It has a 5-way switch. The so called special switch. Any help would be greatly appreciated. Thanks.
