No hacking was needed. He hot linked to a user heres pics so the user changed the pic to that monstrous collage leaving the hot linked URL to the picture valid.

Well, until Ron shares, I will. I was there when it happened.

Location - NAMM: January 2003
There was a group of people standing near the concessions area (IIRC) drinking and whatnot. Ron notices Ed Roman, smiles and says something to the extent of "Hey, there's Ed Roman! Should I go over there, kick him in the ass, and smack him upside the head?" I smiled and said it was a good idea. Satisfied with his answer, Ron nods, turns, and heads over toward Ed.

Sure enough, he slapped him upside the back of his head and kicked him in the butt! I couldn't believe it, but it happened. Ed's wife shook his hand!
