When i see people do this thinking about what they are paying attention to, i think that once they ask the subject to imagine the word on their hand, they watch their eyes and somehow can see the person trying to spell the word with their eyes.

That or they get a vibe from the person, catch a peek at the remaining piece of paper and can scan a page and pick out words that stand out that they feel the person would pick.

My best guess anyway.
